The program is embraced by the government and made sure it is implemented. Health professionals are taught and trained well on the importance of the program, precautionary measures, how to deal with the TB patient, and, most importantly, on the proper identification and reporting of sputum microscopy results. There is therefore uniformity in the reporting of laboratory results. The government has also allotted a specific monetary budget for the services of the health professionals under this program. There is also the regular supply of drugs, not only just drugs but medications that are of good quality and recommended by the WHO. More importantly, these drugs are FREE! All the patient has to do is to comply with the treatment. But, there are still patients who are, well, â€Å"human† – remarkably wise, talented entrepreneur (because some sell their TB medicines to others who also present with TB symptoms as they have). They have a mind of their own; they have their own treatment strategy. These make TB treatment difficult and pose threat on the development of resistant organisms because the medications are not taken on proper dosage and duration. It is because of this that there is this 4th element of the DOTS strategy – the direct observation of the treatment partner (treatment partners could either be a Staff of the Health Center, Member of the Community , or Member of the patient’s family (last priority)) of the patient taking in the TB medications. With good record keeping, the patients are properly monitored and followed up wherever they may be in the country. With just one missed appearance on treatment appointments, the in-charge health partner is alerted. There’s no reason at all for non compliance. What is more wonderful with DOTS in our country is that we have this PPMD or the Private – Public Mixed DOTS. This is very important because even the private health institutions apply DOTS so that wherever may the patient consult he or she is directed to the same treatment strategy for TB which ensures patient receives proper treatment and program benefits. Upon enrollment with the DOTS program, there is an allotted box of medications both for the intensive phase and the maintenance phase – the Stop TB Kit. With those new pulmonary smear positive cases and new seriously ill pulmonary smear negative cases, the intensive treatment is made up of four regimens: Isoniazid, Rifampicin, Pyrazinamide and Ethambutol. The number of capsules taken will depend on the weight of the patient (since usually the four drugs mentioned are formulated on a single capsule). Like for example, a patient weighing between 38-54 kg will be taking 3 tablets each appointment time with the treatment partner. The intensive treatment lasts for 2 months then a sputum smear will be requested. If patient is still sputum AFB smear positive there will be a month extension for the intensive phase. Afterwhich is the 4 months of maintenance phase. The regimen usually is composed of Isoniazid and Rifampicin. The kind and number of treatment regimens for tuberculosis is such because the TB bacilli, although TB is curable, is very difficult to eradicate and are prone to resistance. This is also why compliance is very important to prevent further development of resistant organisms. Whenever there are MDR TB patients (Multi Drug Resistant TB), they are referred to a higher center in Manila. There, patients are housed for 24 months, still free treatment. I think each MDR patient is budgeted with 500 1 million pesos such a huge amount. It is important therefore for us to prevent the development of such resistant organisms if we want our taxes serve the majority and give us better lives. Through the exposures regarding the DOTS, I now have a great appreciation for this program of the government. Whenever I will have an encounter with a possible TB patient, I could confidently refer him to a DOTS facility. I am hoping that we will truly be able to achieve our vision with regards to tuberculosis control as a nation.

Top 17 Exposures Needed to Learn New Words While technically not a muscle, a students brain benefits from regular daily exercise. Where there are health and fitness  experts who design routines and make recommendations for building specific body muscles using  repetition (reps) in sets, there are U.S. Department of Education experts who recommend the  learning of vocabulary through repetition (reps) or exposure to a word. So, just how many repetitions do these education experts say are necessary?  Research shows the optimum number of repetitions for vocabulary to go into the long-term memory of the brain is 17 repetitions. These 17 repetitions must come in a variety of methods over planned periods of time. The Brain Needs 17  Repetitions   Students process information during the school day into their neural network.  The brains neural networks form, store, and re-form information into long-term memory that can be recalled like files on a computer or tablet. In order for a new vocabulary word to make the journey into the brains long term memory, a student must be exposed to the word in timed intervals; 17  timed intervals to be exact. Teachers need to limit the amount of information presented per unit of time and repeat it cyclically throughout the day. That means students should never be given a long list of vocabulary words for one exposure and then be expected to retain the list for a quiz or test months later.  Instead, a small group of vocabulary words should be introduced or explicitly taught for several minutes at the beginning of a class (first exposure) and then revisited, 25-90 minutes later, at the end of class (second exposure). Homework might constitute the third exposure. In this way, over the course of six days, students can be exposed to a group of words for the optimum number of 17 times. The experts from the U.S. Department of Education  also strongly suggest that teachers  dedicate a portion of the regular classroom lesson to explicit vocabulary instruction. Teachers should also vary this explicit instruction by taking advantage of the way the brain learns, and include multiple instruction strategies that are auditory (hear the words) and visual (see the words). Build Vocabulary Muscles Just like a body workout, a brain workout for vocabulary should not be boring. Doing the same activity over and over will not help the brain develop the necessary new neural connections. Teachers should expose students to the same vocabulary words in a variety of ways: visual, audio, tactile, kinesthetic, graphically, and orally.  The list below of 17 different types of exposures follows the design of the  Six Steps for Effective Vocabulary Instruction, a set of recommendations by education researcher Robert Marzano.  These 17 repeated exposures begin with introductory activities and end with games. 1. Have students start with a sort by having them separate out the words in ways that make sense to them. (Ex: words I know vs. words I dont know or words that are nouns, verbs, or adjectives) 2. Provide students with a description, explanation, or example of the new term. (Note: Having students look up words in dictionaries is not useful for teaching vocabulary. If the vocabulary word list is not associated with or taken from a text, try and provide a context for the word or introduce direct experiences that can give students examples of the term.) 3. Tell a story or show a video that integrates the vocabulary word(s). Have students create their own videos using the word(s) to share with others.   4. Ask students to find or create pictures that explain the word(s). Have students create symbols, graphics or comic strips to represent the word(s).   5. Ask students to restate the description, explanation, or example in their own words. According to Marzano, this is an important repetition that must be included. 6. If applicable, use morphology and highlight the prefixes, suffixes, and root words (decoding) that will help students remember the meaning of the word. 7.  Have students create lists of synonyms and antonyms for the word. (Note: Students can combine #4, #5, #6, #7 into the Frayer model, a  four-square graphic organizer for building student vocabulary.) 8. Offer incomplete analogies for students to complete or allow students to write (or draw) their own analogies. (Ex: Medicine:illness as law:_________). 9. Have students engage in conversation using vocabulary words. Students can be in pairs to share and discuss their definitions  (Think-Pair-Share). This is particularly important for EL students who need to develop speaking and listening skills. 10. Have students create a concept map or  graphic organizer that has students draw an illustration representing vocabulary words to help them think about related concepts and examples. 11. Develop word walls that display vocabulary words in different ways.  Word walls are more effective when they are interactive, with words that can be easily added, removed or rearranged. Use pocket charts, or index cards with peel-and-stick Velcro, or peel-and-stick magnetic strips. 12. Have students use the activities on mobile vocabulary apps: Quizlet; IntelliVocab for SAT, etc. 13.  Cover a wall with paper and have students create  word posters or graffiti  the walls with vocabulary scribbles. 14. Create crossword puzzles or have student design their own crossword puzzles (free software programs available) using vocabulary words. 15. Have students interview a word by  teams as a class or small group activity. Give one team a word and list of interview questions. The average call center graduate, usually holding a bachelor’s degree (BA) earns 3,000 rupees a week, which is the equivalent of 67 U. S. dollars (Aspden, 2006). To facilitate the need to off shore, construction of office space is taking 10 months to complete instead of the normal 18 months in Bangalore, India. Each month over 40 companies make the decision to outsource Information Technology (IT), back-office and customer service departments to India, and other countries. To fight the onslaught of new business, India has setup second tier cities to handle the overflow of demand and establish call centers such as Pune, Kolkata, and Jaipur, just to name a few. These cities in India are growing quickly to accommodate the need for outsourcing, while slowly loosing the foothold on the market due to the increases in pay. With other countries such as China, Hungary, and the Philippines competing for lower wage work, India may loose almost half it’s market share by 2007 (Aspden, 2006). However, companies are finding out that there are hidden costs involved with the outsourcing of customer service. A growing trend of customer dissatisfaction is causing major corporations to reconsider the outsourcing of its services. In a recent survey of pension policy holders in the United Kingdom (U. K. ) it was found that 75% would leave their current provider if they experienced bad customer service (Pfeffer, 2006). Mega corporations such as Dell, Capitol One, and JP Morgan all made an attempt to outsource customer service and all reversed course. Evidence is mounting that good customer service is pivotal in a company’s retention of customers. A 2005 Gartner study predicted that 60% of organizations that outsource customer-facing processes will see significant numbers of frustrated customers switching to competitors (Pfeffer, 2006). Even call centers in the United Kingdom (U. K. ) have reverted calls back to their own soil due to customer complaints, with the majority of those companies vowing to keep call centers in the U. K. (Kivlehan, 2005). The corporate mentality towards outsourcing is far understated as companies see the business case for the moves, assuming that customers do not pay attention to dialect and local customs. After years of wondering what all those fiber-optic cables laid around the earth at massive expense in the late 1990s would ever be good for, we finally have an answer: They are good for enabling call-center workers in Bangalore or Delhi to sound as if they are next door to everyone. Furthermore, micromanagers do not instill leadership qualities in their subordinates. This could result in the creation of a hostile work environment between subordinates and leaders because employees do not have the appropriate space to learn and grow. A more dangerous aspect of micromanagement is being abusive towards employees. Autocratic managers should be cautious not to issue threats, yell or use abusive language towards their employees. Although considered the least desirable by most subordinates and managers alike, there is a time and a place for the directing style.
